In addition to a personalized care plan specifically tailored to your loved one’s daily needs, we employ innovative and effective programming designed to enhance connection, build strength and inspire confidence.
At Amira Choice Forest Lake, our management partner, Ebenezer, provides our memory care residents with its Dimensions® program, a multifaceted memory care program that ensures residents receive exceptional care based on current best practices. It emphasizes relationship-based care, nutrition and wellness, creative engagement, calm environments, best-in-class training to staff and support to families.
We also support the families of our residents living with memory loss through a variety of care resources, current research and activities designed to bring our Forest Lake, Minnesota, community together. Our memory care neighborhood is small and easy to navigate, offering residents a greater sense of independence as they explore, spend time with friends and neighbors and take part in activities and events. A large enclosed outdoor patio and garden provide a lovely spot to enjoy the sunshine and fresh air. To fully support our residents’ nutritional needs, we provide three chef-prepared meals daily in our dining room and always have a variety of snacks on hand.

We welcome residents of all faiths. We offer non-denominational church services and regular Catholic Communion.
Engaging Programming
You’ll have no trouble keeping busy with our full calendar of activity programs. Our social events, fitness classes, scheduled group outings, guest entertainment, clubs and more are all arranged to fit the interest of our residents.
Visiting Health Providers
We partner with provider groups to bring experienced primary care doctors and nurse practitioners, podiatrists and other specialists directly to our community on a regular basis. In addition, we partner with Medicare-certified providers to offer covered nursing services; physical, occupational and speech therapies; and hospice and social work services.
Care Services
Full-time-licensed nurses and 24-hour personal care assistants are available to all residents receiving care. Residents can choose from a large menu of supportive homemaking and healthcare services, like assistance with dressing, bathing, medication management and more. Our list of available services is extensive and includes two-person and mechanical lift transfers, assistance with insulin and other medical needs.

Just reach out! Talk with our team about apartment availability. If your preferred floor plan is available, we will gladly work with you on the next steps in the move-in process. If the apartment of your choice is not available, you can put down a refundable deposit to be added to our waitlist for that specific unit.
If you (or your loved one) aren’t quite ready for senior living, but feel the time is coming soon, don’t hesitate to visit us. The sooner you get on the waitlist for the specific type of floor plan or view that you want, the more options you will have when the time comes for a move.
Your memory care rent includes a bright apartment home with a private bathroom. The monthly rent covers utilities such as electric, gas, water, trash removal, recycling, basic cable TV and guest-shared wireless internet. Our on-site team takes care of apartment maintenance and repairs.
Access to our many community amenities located outside the memory care neighborhood are also included in the rent. For safety and support, residents living in memory care enjoy these amenities with the accompaniment of a staff or family escort.
At Amira Choice, apartments are available for rent. Our leases are month-to-month, so there are no long-term commitments.
Your Medicare or health insurance does not pay for senior living rent. However, it may help pay for some of the services you receive while living in our community. Some residents have long-term care insurance plans that may help reimburse them for some of the day-to-day expenses of Assisted Living or Memory Care.
Amira Choice Forest Lake accepts a limited number of residents receiving Medicaid covered nursing services, assistance with activities of daily living and other services. Please contact us for additional information.
You will need a doctor’s order to move into memory care. When you are ready to move in, our healthcare team will provide an assessment, discuss available services that may be beneficial to you and be there to coordinate with your doctor every step of the way.
Yes, you may work with any healthcare provider that you choose. You can continue to go out to your doctors’ appointments as you have been doing. There are also doctors and other healthcare teams that visit our community.
We provide residences and care for older adults of all faiths. You do not need to be any specific religion to live in our community.
Absolutely! Your family and friends are welcome to visit you anytime. We encourage your family members and friends to be involved in the life of our community.
Visiting pets are welcome, but pets are not permitted to live with residents in memory care.
